TERMS AND CONDITIONS

• All Quotations are valid for 14 days only
• To make bookings and to accept the quoted approximate prices please fill in our booking form on www.allaboutmemakeovers.com  or request a form to be mailed or faxed to you.
• Disclaimer must be signed and returned by fax or email before booking goes ahead.
• Our quotation for your surgery is an approximation only as pricing depends on current exchange rates and to the general health of the patient.
• A full quote will be given once you have had your  initial consultation with your surgeon
• The staff and representatives of All About Me Holiday Makeovers are not medical practitioners.  We are the co-ordinators of your medical package.  The hospital is responsible for all the medical advice which is directed through All About Me Holiday Makeovers as their representative Agents.
• Please try to allow at least 7 days after surgery.  For multiple surgeries, which are quite common, please try to allow for 12 -14 days.
• You may be asked to supply photos so that the Doctor can pre-plan your surgery more accurately.  This is advisable for large areas and multiple surgeries especially
• All About Me Holiday Makeovers will proceed with your booking and obtain your Hospital Registration Number on receipt of the Booking Agents fee as requested.  Payment of the booking agent fees to be made payable to -- All About Me Holiday Makeovers by -- Cash, cash deposit; bank transfer or, cheque made out to All About Me Holiday Makeovers.  
• A fee of approximately 2200THB ($65.00) is payable on day of your Doctors consultation.  This is for pre-op test.
• You will be advised by your coordinator, how and when to pay your Surgery package fees.  These fees are paid directly to the hospital.
• PLEASE NOTE:  After All  About  Me Holiday Makeovers has booked your surgery  and obtained all your hospital details if at any point there-on–in  you then decide not to go ahead with your surgery.
• PLEASE NOTE: our Booking Agents fee of AUD $395.00  is non-refundable

YOUR SURGERY WILL EXCLUDE:

• Normal routine pre-op does not cover Mammograms or EKG’s or other specialised tests.  Women over 40 will require a mammogram if not supplied. (approx. cost 3000THB)
• Any treatments for underlying diseases or conditions, medication or laboratory investigations, that could arise during the normal pre-op investigations
• Heart, lungs, kidney, liver malfunctions before or during operation that will require urgent treatment.
• Extra hospital room charges for patient personal expenses.
  Including: room upgrade, extra meals, extra accommodation for stays longer than package price stated.
  Soft drinks papers, phone calls from hospital phone.
• Any additional charges for treatments, incurring costs after the operation. Or any other Treatments or medical  costs not related to the original package price.

NOTE:

• The Doctor may request photos so as to be able to evaluate a patient’s condition more accurately.  This is especially so for patients wanting multiple surgeries.
• Patients must disclose all diseases or blood disorders in their medical questionnaire. Smoking or the taking of any Aspirin or Aspro related medication must be disclosed, as they can lead to severe bleeding.
• Packages deals and prices are subject to changes without prior notice.
